Patient Self-Care

Integrated care achieves best results when patients take control of their own health - when they actively manage their own care, avoid unhealthy behaviors, and can accurately identify when they need clinical intervention.

The "Panel Approach"

Integrated care is provided by a team of professionals who must work together to deliver necessary services.  Each member of the care team understands what they are accountable for, develop and agree on protocols for how your care will be delivered, and communicate regularly with each other to ensure efficient care and information sharing.
